We will provide a pricing list with all of the services, and their associated costs. You will be able to make an informed decision prior to agreeing to have any procedure performed. Payment is due at the time of service.
Happy Feet Podiatry LLC currently participates with the insurance plans listed on the right. If you are not covered by these insurance plans, but you have out-of-network benefits, your insurance will typically cover your charges. Please check with your insurance carrier before your first appointment. In some cases, patients with out-of-network insurance plans will be treated as SELF PAY PATIENTS until payment is secured from the Insurance Company, and a refund will be given to the patient as necessary.
The patient is responsible for securing a referral, if required, prior to their first appointment with Happy Feet Podiatry LLC.
Many insurance plans cover the dispensing of custom orthotics for medical necessity (reference Services for a description of Orthotics). If your insurance does not cover orthotics, our office charges $400.00 for the casting (making an impression of your feet), fabrication and dispensing of custom orthotics. A $200 deposit is required at the time of casting, and the final $200 is due at the time of dispensing.
